Gynaecomastia is the medical term for the large breast in adolescents and men, which is related to the excessive increase in size of the gland (breast). During adolescence, it is normal for the breast to enlarge, this normally resolves in the early years of adulthood.
Most of the time gynaecomastia is idiopathic, so, there is no identifiable cause. However, it may sometimes be due to a hormonal disorder. Signs and symptoms become more intense if we use anabolic steroids or other medicines.
The breast consists of fat, which is soft and fibrous tissue, which is hard. The percentage of fat and hard fibrous tissue varies. The breast growth may affect one breast or both.
Treatment of idiopathic gynaecomastia is performed surgically and medication has no effect and is not necessary. The surgery may include, depending on tissue composition and fat content, liposuction, excision of the gland, or a combination of the above.
Liposuction involves dissolving fat from the breast gland and the chest wall with small cannulae, through 1-2 incisions of 5 millimeters. Subsequently, we make a small incision in the lower half of the periareola, from where we excise the gland.
The incisions, although permanent, usually heal quickly and well and are difficult to see after a few months.
The difference and improvement after surgery appears to be immediate, although the final result may take 3-6 months.

How is diagnosis made and what are the conditions for surgery?
The diagnosis of gynaecomastia is done by clinical examination and the decision for surgery is based solely on the size of the gland, the presence of fat on the breast and the chest and how close we are to our normal weight.

What kind of anesthesia is required?
The surgery is usually done under general anesthesia and takes about 2 hours.

What care do I need after surgery?
After surgery, we wear a light pressure corset on the chest for 3 months.

How long will it take to recover?
In the first 2 weeks we may have sensitivity in the area and need to take analgesia. After the first 2 weeks we completely remove the dressings but continue with the corset.
